Follow these instructions to take the necessary photos for submitting a request to have your aligner tracking assessed virtually and receive your next set of aligners.

– Wash your hands thoroughly for at least 20 seconds.

– Find a well-lit room in your house. One with a lot of natural light is best where the light is facing you.

– Take the following 5 photos with your phone or camera. Make sure the flash is turned on. If you see a shadow over your teeth or if they are out of focus, please take them again. You may need to recruit a friend to help you.

– Use the form below, text the 6 photos 503-399-0721 or Email along with your name, date of birth, and elastics info.
